Facility Purpose and Mission

Cultivating Youth Transition Home provides a safe, structured, and supportive environment for formerly incarcerated individuals who are reentering the community. The home’s mission is to promote successful reintegration, reduce recidivism, and support residents in achieving stability through employment, education, and life skills development. The program focuses on building independence while offering 24-hour supervision and access to supportive care services.​​

What is Transitional Housing?

This is a non-medical, supportive housing facility offering 24-hour care and supervision. Services provided include life skills and job-readiness training, case management and re-entry planning, assistance with employment and housing applications, behavioral and emotional support through referrals to licensed providers, transportation assistance for appointments, work, and court obligations, and connection to substance use recovery and counseling programs. Care focuses on structure, accountability, and daily support rather than medical or clinical treatment. 

Home Details 

Layout: Shared & communal living space, bedrooms, bathroom, living room, kitchen & dining, laundry room, parking and outdoor yard space. 

Target Population: Adult males, ages 18 and older, who are formerly incarcerated and currently on probation or parole.

Fee: There is no cost to residents of the transitional home.

Prohibited: Sex Offenders
